29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:  On or about March 11, 2020, the employer failed to ensure that a periodic inspection was conducted of the energy control procedures when the service technician employees performed lock out during maintenance of equipment including, but not limited to the dynamometer machines.

29 CFR 1910.147(c)(7)(iv): The employer did not certify that employee training had been accomplished and kept up to date:  On or about March 11, 2020, the employer failed to certify that employee lock out training had been accomplished and kept up to date for service employees who perform maintenance on equipment including, but not limited to the dynamometer machine. The employer states that training occurred at the time of initial hire.
