29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems or a combination of warning line system and safety monitoring system.   On or about March 12, 2020, employees performing roofing operations were not protected from a fall, exposing employees to a fall of over 25 feet.   Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.    On or about March 12, 2020, employees used an extension ladder where the length did not allow for a three foot extension past the upper landing, exposing employees to the hazards associated with improper ladder use.   Poulokefalos Enterprises LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.1053(b)(1), which was contained in OSHA inspection number1134578, citation number 1, item number 3a and was affirmed as a final order on June 1, 2016, with respect to a workplace located at 150 Gaylord St. Elk Grove Village, IL 60007.  Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).
