(b) On the roof - On or about March 11, 2020 an employee was exposed to flying objects while using a nail gun without wearing safety glasses.

29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift: (a) In front of the building on to the roof - On or about March 11, 2020 an employee was exposed to a fall while working from a JLG460SJ Telescopic Boom Lift that was elevated 18 feet 2.5 inches above the ground with no fall protection. (b) In front of the building - On or about March 11, 2020 a second employee moved the aerial lift with no fall protection. The employer did not provide harnesses or lanyards for employees working inside an aerial lift.
