29 CFR 1910.147(c)(6)(i)(C): Where lockout was used for energy control, the periodic inspection did not include a review, between the inspector and each authorized employee, of that employee's responsibilities under the energy control procedure being inspected: An employee who performed work requiring lockout, bandsaw blade change, was not included in the annual review for each authorized employee performing lockout work to review how lockout work was to be conducted. Abatement documentation is required for this violation.

29 CFR 1910.147(c)(4)(ii)(B): The energy control procedures did not clearly and specifically outline the steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy: The lockout specific procedure for the Powermatic band saw incorrectly stated that de-energization required "unplug and lockout." The machine could not be unplugged, but would be locked out at the machine disconnect. Abatement documentation is required for this violation.

29 CFR 1910.95(d)(1)(i): The sampling strategy was not design to identify employees for the inclusion in the hearing conservation program and to enable the proper selection of hearing protection: Noise monitoring had not identified a punch press operator that was likely to be exposed to noise levels exceeding the action level of 85 dBA. (The operator consistently wore hearing protection.) Abatement documentation is required for this violation.
