29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:      a) Job site, 5180 Campbells Run Road, Pittsburgh, PA: On or about March 12, 2020, during roofing activities, the employer did not ensure that employees used appropriate fall protection systems/devices where exposed to fall hazards of up to approximately 19 feet.

29 CFR 1926.1053(b)(16): Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were permitted to be used:   a) Job site, 5180 Campbells Run Road, Pittsburgh, PA: On or about March 12, 2020, during roofing activities, the employer did not ensure that the ladder at the site was safe to use in that it had damaged rungs, some of which had large tears in the aluminum that it was made from.
