29 CFR 1926.1053(b)(16): Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were permitted to be used: a) Job site, 5180 Campbells Run Road, Pittsburgh, PA: On or about March 12, 2020, during roofing activities, the employer did not ensure that the ladder at the site was safe to use in that it had damaged rungs, some of which had large tears in the aluminum that it was made from.
