29 CFR 1910.147(c)(4)(ii)(B): The energy control procedures did not clearly and specifically outline the steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy:    a)  America Flux and Metal LLC.- Throughout the production Area: The energy control procedures were not specific for each individual type of machine (machines include but are not limited to: Spray Dry machine, Hydrogen Furnaces, Blender, Pellet Press, Bucket elevator, etc.).  The program only has a one fits all LO/TO procedure and it did not clearly and specifically identify the placement, removal or transfer of Lockout /Tagout devices on each individual type of machine and did not specify energy sources present and the requirements for testing to verify the effectiveness of LO/TO devices and other control measures. Occurred on or about 03/12/2020.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    a) America Flux and Metal LLC. - Reduction Room:  The Furnace #1 and Furnace #2 have unguarded sides where employees are exposed to severe burns, The Oxide Blender has two pinch points, the Pellet Press Sweco is operated with an open guard at the point of operation, observed on or about 03/12/2020.    b) America Flux and Metal LLC. - Induction Room:  The deck furnace is not guarded to prevent operators accidental exposure to burns while the furnace is in operation, observed on or about 03/12/2020.
