29 CFR 1910.110(e)(4)(iii): Permanent and removable fuel containers were not securely mounted to prevent jarring loose, slipping, or rotating. On or about March 17, 2020, the employer did not ensure that the removable LP fuel container were securely mounted on the powered industrial vehicle (forklift truck). The forklift truck was used in operation without the necessary indexing pin positioned in a manner to prevent displacement of the container. Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).

The Komatsu forklift truck was used in operation without the necessary indexing pin positioned in a manner to prevent displacement of the lp fuel container. Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).
