On or about March 17, 2020, located off of 848 Peacock Drive in Sunbury, Ohio, the employer did not ensure that an employee had an immediate and safe means of egress while working inside of a trench shield, thereby exposing the employee to the hazards associated with trenching during an emergency situation. i.e. cave-in.

On or about March 17, 2020, located off of 848 Peacock Drive in Sunbury, Ohio, an employee was exposed to a cave-in hazard while performing pipe laying work in an approximately seven and a half to eight foot excavated trench. The employer did not ensure that the employee in the trench was protected at the excavator end of the trench shield.
