29 CFR 1926.652(d)(2): Manufactured materials and equipment used for protective systems shall be used and maintained in a manner that is consistent with the recommendations of the manufacturer, and in a manner that will prevent employee exposure to hazards.  a. On or about March 17, 2020, located at 1187 Genoa Baptist Church at 7562 Lewis Center Road in Westerville, Ohio, an employee was working in a GME trench shield, model number 4M820, serial number M01111930F-2. The trench shield was not used and maintained in a manner consistent with the recommendations of GME as there was one spreader pin missing and another not fully inserted to secure the spreaders in place.
