29 CFR 1926.502(d)(6)(i): Snap hooks not designed to be connected directly to webbing, rope or wire rope were used:   Location: roof building #4 On or about 2/24/20, employees failed to ensure employees using personal fall arrest systems were properly anchored in that their ropes were wrapped around a wood truss and then hooked back to the rope.
