29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents. On the west side of the construction site, employees engaged in decking work used pneumatic nail guns and were not wearing eye and face protection. The employees were exposed to struck-by hazards and eye injuries.

On the west side of the construction site, employees engaged in decking activities on a residential home from a single pole scaffold worked from a platform that was not 18 inches wide exposing employees to a fall hazard approximately 14 feet to the ground below.
