29 CFR 1926.453(b)(2)(v): A safety harness with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:    a) Disability Determination Division, 2550 Prospect Avenue, Helena, MT: On or about March 17, 2020 and at times prior, employees were exposed to catapult and subsequent fall hazards while operating a Genie Aerial Lift, model Z-34/22, serial # 12938 to gain access to the roof from an approximate height of 15 feet without wearing fall protection.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-sloped roofs with unprotected sides and edges 6 feet (1.8m) or  more above lower levels was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line systems and guardrail systems, warning line systems and safety net systems, or warning line systems and personal fall arrest systems, or warning line systems and safety monitoring systems:    a) Disability Determination Division, 2550 Prospect Avenue, Helena, MT: On or about March 17, 2020 and at times prior, employees were exposed to fall hazard approximate height of approximately 15 feet while applying the metal edge to the front of the roof without fall protection.
