29 CFR 1910.147(c)(5)(i): Locks, tags, chains, wedges, key blocks, adapter pins, self-locking fasteners, or other hardware were not provided by the employer for isolating, securing or blocking of machines or equipment from energy sources:  a) On or about March 13, 2020 and times prior;  lockout equipment not provided for operators to lockout press #69: The employer failed to properly identify press operators as authorized lockout employees and provided them with lock(s), tag(s), hasp and other needed equipment to safety lockout energy sources such as electrical, pneumatic, and gravity while conducting a die change on press #69.

29 CFR 1910.147(c)(7)(i): The employer did not provide adequate training to ensure that the purpose and function of the energy control program was understood by employees:  a) On or about March 13, 2020 and times prior;  Lockout/Tagout (LO/TO) training: The employer failed to ensure operators who perform die changes on press #69, were trained on LO/TO procedures to enable operators to understand  the hazards associated with 480v electrical, 120psi pneumatic, and gravity from the ram and die, to enable them to safely lockout the energy sources before working on the press.
