29 CFR 1903.19(c)(1): The employer failed to certify to OSHA that the following item(s) were abated within 10 calendar days after the abatement date:    On or about June 01,2020, the employer failed to certify to OSHA that the following hazards have been abated:  a. Citation 1 item 2: On or about 03/26/2020, employees installing plywood sheathing to a 5:12 pitch roof on a newly constructed residential property locate at Lot #4, Electric Ave. in Mableton, GA 30168, were not protected from a fall hazard by conventional fall protection and/or devices. Employees were exposed to a fall hazard from a height of approximately 19-feet, 10-inches from eave height to ground level.

29 CFR 1903.19(d)(1): The employer did not submit to the Agency documents demonstrating that abatement is complete for each willful or repeat violation and for any serious violation for which the Agency indicates in the citation that such abatement documentation is required:  On or about June 01,2020, the employer failed to provide documentation to OSHA demonstrating that the following hazards have been abated:  a. Citation 1 item 2: On or about 03/26/2020, and at times prior, the employer exposed employees to fall hazards of 19-feet, 10-inches to the lower level, in that a fall protection system was not provided to employees engaged in plywood sheathing installation on a 5/12 pitched roof.

29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles:  a) New residential construction located on Lot #4, Roof: On or about 03/26/2020, and at times prior, the employer exposed employees to struck-by hazards while operating pneumatic nail guns, in that protective eyewear was not provided to employees engaged in plywood sheathing operating these tools at 120 PSI.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501(b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:   a) New residential construction located on Lot #4, Roof: On or about 03/26/2020, and at times prior, the employer exposed employees to fall hazards of 19-feet, 10-inches to the lower level, in that a fall protection system was not provided to employees engaged in plywood sheathing installation on a 5/12 pitched roof.
