29 CFR 1926.405(j)(1)(ii): Fixtures, lampholders, rosettes, or receptacles were not securely supported: On or about April 1, 2020, at a job site located at 6200 Landerhaven Dr, Mayfield Heights, Ohio 44124; during the construction of commercial assisted living facility, the employer did not ensure all employees were protected from electric shock be ensuring that all receptacles were supported in the main hallway which had multiple employees exposed.

29 CFR 1926.416(a)(1): Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means: On or about April 1, 2020, at a job site located at 6200 Landerhaven Dr, Mayfield Heights, Ohio 44124; during the construction of commercial assisted living facility, the employer did not ensure all employees that were working and traversing were protected from electric shock from exposed live wires that were located directly in the path of travel as you entered the main access/egress door on the Northeast Side of the project. More than 12 employees access the work location by entering the structure through that point of access exposing the employees to the hazard.
