29 CFR 1926.252(a): An enclosed chute of wood, or equivalent material, was not used where materials were dropped more than 20 feet to points lying outside the exterior walls of the building(s):  Worksite located at N Haven, Liberty MO -- Employees were discarding materials, including scrap lumber/sheets of plywood, from the roof in excess of 25 feet above ground level without an enclosed chute or equivalent. This exposed employees to struck by/crush by hazards. This was documented on or about 03/31/2020.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  Worksite located at N Haven, Liberty MO -- Employees were roofing a hotel building, commercial property, with no fall protection, guardrails or safety net systems. Employees were exposed to fall hazards. This was documented on or about 03/31/2020.
