29 CFR 1910.305(b)(2)(i): Each outlet box in completed installations did not have a cover, faceplate, or fixture canopy:  a) L&M Machine, top brace above saw blades, Fox Lumber Sales, Inc.: On or about April 2, 2020, and at times prior, employees were exposed to electrical shock and fire hazards while working around exposed wires coming from a live junction box. The junction box had exposed wires from the missing cover.   b) Area where the door to the chipper/mulcher is located, behind L&M Machine, southwest corner, Fox Lumber Sales, Inc.: On or about April 2, 2020, and at times prior, employees were exposed to electric shock and fire hazards while working around exposed wires coming from a live junction box. The junction box had exposed wires from the missing faceplate.  c) Line II MCC B, Fox Lumber Sales, Inc.: On or about April 2, 2020, and at times prior, employees were exposed to electrical shock and fire hazards while working in and around an energized MCC B 480 volt panel. The MCC B panel had exposed wiring behind the missing instrumentation.      d) Panel for Multi Blade Saw, Fox Lumber Sales, Inc.: On or about April 2, 2020, and at times prior, employees were exposed to electrical shock and fire hazards while using energized multi blade saw. The multi blade saw panel had exposed wiring from the missing lower section of the cover.    e) Recovery System Main Disconnect, Fox Lumber Sales, Inc.: On or about April 2, 2020, and at times prior, employees were exposed to electrical shock and fire hazards while working in and around the electrical conductors for the main disconnect to the recovery system. The main disconnect had exposed wiring from the missing cover.
