29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered:      On or about February 12, 2020, the employer required employees to fill powered industrial vehicle batteries with distilled water without providing employees with personal protective equipment such as, but not limited to, goggles and rubber apron to prevent injuries from splash hazards.    Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:    On or about February 12, 2020, the employer required employees to fill powered industrial vehicle batteries with distilled water without having an eyewash readily accessible to prevent injuries due to splash hazards.    Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.178(l)(1)(ii): The employer did not ensure that each operator had successfully completed the training required by paragraph (l), except as permitted by paragraph (l)(5), prior to permitting an employee to operate a powered industrial truck    On or about February 12, 2020, the employer did not provide practical hands-on training or formal training such as, but not limited to lectures, discussions, interactive computer learning, video tapes, or written materials to employees required to operate powered industrial vehicles to eliminate or reduce stuck-by hazards.    Abatement certification is required for this item in accordance with the requirement of 29 CFR 1903.19(c).
