Fort Recovery, OH: The employer did not ensure a safe means of egress such as a ladder was in place in the 7 ft. deep trench in which employees were working.

(a)  Shinn Bros., Inc. on or about April 8, 2020, at the site located near the address of 284 Railroad Street in. Fort Recovery, OH:  The employer failed to ensure employees were adequately protected from cave-in hazards while performing work activities in approximately a 7 ft. deep trench.
