29 CFR 1910.212(a)(3)(ii): Point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle:  a) Machine shop section: On or about April 7, 2020, and at times prior, the employer exposed employees to amputation hazards in that the point of operation for rotating parts on equipment such as, but not limited to the drill press was not guarded while employees were customizing tent pole and tent pole fittings.

Self-luminous or electroluminescent signs that have a minimum luminance surface value of at least .06 footlamberts (0.21 cd/m2) are permitted.  a) Shipping area: On or about April 7, 2020, and at times prior, the employer exposed employees to fire hazards, in that, the employer failed to have an illuminated exit sign for the shipping area emergency exit door to assist employees with egress in the event of an emergency.
