29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:  A) At the job site, the employer did not instruct employees performing drilling of well points using a Takeuchi TB285 hydraulic excavator to avoid contact with overhead power lines energized at 7.6 kV, exposing employees to electrocution hazards, on or about April 13, 2020.

29 CFR 1926.416(a)(1): Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means:  a. On or about April 13, 2020, at the W. Lake Eloise Drive Roadway project, employees installing a dewatering system in close proximity to an overhead power line energized at 7.6 kV were exposed to an electrocution hazard.
