29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a) Seyekcub Inc, located at 615 West 4th Street, Uhrichsville, OH 44683: On and preceding April 20, 2020,  the employer did not ensure that one or more methods of machine guarding was provided to protect the operator and other employees in the machine area from hazards created by a mixer/auger blade. Employees utilize a Palmer Manufacturing M500XL high speed continuous mixer as part of daily operations in the facility.  The outlet/chute which discharges the sand mixture had an opening that was 6 inches in diameter which was 14-15 inches from the blade.  The following condition would allow employees to reach into the outlet and contact the blade, resulting in a caught-in/between injury.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:  a) Seyekcub Inc, located at 615 West 4th Street, Uhrichsville, OH 44683: On and preceding April 20, 2020, the employer did not ensure that employees were provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment.  Employees exposed to chemicals which include but are not limited to a two part phenolic urethane binder were not provided information and training on the chemicals when assigned duties exposing them to the chemicals.
