Location: 3432 Atlantic Ave, Brooklyn, NY.  On or about 4/25/2020,  a) Lockout/tagout procedures were not utilized by employees when performing maintenance on pressurized air suspension system of a vehicle.  Employees were repairing a pressurized air suspension system.  A piston from the suspension system dislodged and struck an employee.
