The employer did not develop and implement a written respiratory protection program, on or about 4/16/20, and thereafter.

The employer did not provide medical evaluations to determine employees' abilities to use respirators, on or about 4/16/20, and thereafter.

The employer did not conduct fit testing prior to initial use of 3M N95 respirators for affected employees, on or about 4/16/20, and thereafter.
