29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following:  Guardrail systems, safety net systems, or personal fall arrest systems:    a)  On or about April 9, 2020, an employee operating a Toyota Order Picker in the warehouse to pick orders in aisle 12 without the use of a personal fall arrest system or other fall protection was exposed to a fall of approximate twenty (20) feet, thereby exposing the employer to the hazards associated with falls.

29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours.     (a) On or about April 9, 2020, the employer did not contact OSHA within 24 hours when an employee sustained a work-related injury, fall from an order picker, that resulted in an in-patient hospitalization.
