29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.       On the south side of the construction site, an employee on top of the commercial building was straddling the parapet without fall protection retrieving AC and heating material and was exposed to an unprotected fall hazard of approximately 16 feet to the ground below.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed.        On the construction site, employees use an extension ladder to access and egress the roof area and it did not extend 3 feet above the landing exposing them to potential fall hazards.

29 CFR 1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would enable each employee to recognize hazards related to ladders and stairways and train each employee in the procedures to be followed to minimize these hazards.   On the construction site, employees used an extension ladder to access and egress the roof area and was not provided hazard recognition by a training program to recognize and avoid ladder hazards.
