29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c).  a) The Service People, Inc. located at  20 S. Hempstead Road Westerville, OH 43081:  On and preceding May 7, 2020, the employer did not ensure that employees were protected from the hazard of cave-ins by an adequate protective system. Employees were performing sewer line repair/installation in an excavation that was 8 feet deep.  Work was being performed at the bottom of an unprotected vertical face of the trench that was approximately 18 inches outside of the shoring on both ends of the trench.  The following condition exposed employees to struck-by/caught-in hazards.
