Carpenters were installing roof sheathing while using a powered nail gun and circular saw. The employees were not utilizing eye protection, which exposed them to struck-by hazards to the eyes.

The path to ground from circuits, equipment, and enclosures shall be permanent and continuous. a) Gustavo Adolfo Beltran Rodriguez, at a jobsite located at 1063 Ravine View Drive, Galena, Ohio 43201: On and/or preceding May 7, 2020, the employer did not ensure employees who used extension cords were protected from electrical contact hazards. Framers were using an extension cord with the ground prong missing from the male end of the cord, which exposed the employees to a 125 volt electrical contact hazard.

Employees were sheathing the second story roof without any means of fall protection, thereby exposed to fall hazards of up to 30 feet.
