The right button of the two-hand actuators for the press was not properly guarded against unintended operation with a ring guard or similar equipment.

29 CFR 1910.217(e)(1)(ii)(A): Inspections and tests of each press on a weekly basis were not being completed to determine the condition of the clutch/ brake mechanism, anti repeat feature and single stroke mechanism:      A)     Riverside Manufacturing, LLC, located at 2309 Industrial Parkway, Sidney, OH 45365: On or about April 1, 2020, the employer did not ensure that the Bliss 35-Ton mechanical power press (Serial # G-9770; RM Asset # 1052) was inspected and tested at least once a week to determine the condition of the clutch/brake mechanism, anti-repeat feature, and single stroke mechanism. Employees were exposed to struck-by and caught-in-between hazards that could result in serious injuries.
