29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:    (a) Jobsite, Hawthorne Commons, Linden Court, Seven Fields, PA:  On or about May 13, 2020 an employee working on the ground below the area where debris was being thrown from the roof was not wearing a protective helmet to protect them from potential head injury from falling objects.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:    (a) Jobsite, Hawthorne Commons, Linden Court, Seven Fields, PA:  On or about May 13, 2020 employees working on an 8/12 pitch roof were not protected from falling approximately 10 feet by guardrail systems, safety nets or personal fall arrest systems.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  (a) Jobsite, Hawthorne Commons, Linden Court, Seven Fields, PA:  On or about May 13, 2020 the employer did not provide training on fall hazards and the methods to controls such hazards to all employees performing roofing work and exposed to falls.
