29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:   On or about May 12, 2020, during the installation of roofing material at the job site located at 18465 Walnut Dr, Strongsville, Ohio 44149; the employer did not ensure employees working on the roof of a single family home were protected from possible danger of eye injury from impact, or from flying objects from the employees installing roofing material.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   On or about May 12, 2020, during the installation of roofing material at a job site located at 18465 Walnut Dr, Strongsville, Ohio 44149; the employer did not ensure employees working on a roof of a single family home were protected from a fall of approximately 14.6 feet by utilizing a fall protection system.
