29 CFR 1910.147(c)(4)(ii): The procedures shall clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, the following: 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D)  a) On or about 5/14/2020 the employer did not provide machine specific lockout/tagout procedures for authorized employees to utilize that clearly informed authorized employees how to control hazardous energy on equipment such as but not limited to; PACMAC bagging machine (model #9500) that includes; A) A specific statement of the intended use of the procedure;  B)  Specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy;  C) Specific procedural steps for the placement, removal and transfer of lockout devices or tagout devices and the responsibility for them; and  D) Specific requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control measures.

29 CFR 1910.147(c)(6)(ii): The employer shall certify that the periodic inspections have been performed. The certification shall identify the machine or equipment on which the energy control procedure was being utilized, the date of the inspection, the employees included in the inspection, and the person performing the inspection.  a) On or about May 14, 2020, located in the bagging department, the employer did not certify that the periodic inspection had been conducted for the energy control procedure for equipment such as but not limited to; PACMAC Bagging machine (model #9500).

29 CFR 1910.147(c)(7)(i)(A): Each authorized employee shall receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control.  a) On or about 5/14/2020 the employer did not ensure that each authorized employee was trained to recognize the hazards associated with clearing jams on equipment such as but not limited to; PACMAC bagging machine (model #9500) without following energy isolation procedures.   The JCG Industries, Inc., DBA Koch Foods Poultry, Inc. was previously cited for a violation of this Occupational Safety and Health standard  1910.147(c)(7)(i)(A) which was contained in OSHA inspection number 1073993, citation number 1, item number 1 and was affirmed as a final order on 1/7/2016, with respect to a workplace located at 4404 W. Berteau Ave., Chicago IL. 60641.
