29 CFR 1926.300(b)(3): One or more methods of machine guarding were not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks.        On or about May 26, 2020, and at times prior thereto, employees operated a Victaulic Grooving machine and the employer did not ensure a guard or a barrier between the employee and the role grooving rotating parts, exposing employees to amputation hazards.

29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous.      On or about May 26, 2020, the orange electrical cord energizing the Victaulic grooving machine had no grounding pin, making the path to ground not permanent and continuous exposing employees to electrical shock hazards.
