29 CFR 1926.651(c)(2):  The employer did not ensure that a means of egress from trench excavations. A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that was more than 4 feet (1.22 m) in depth so as to require no more than 25 feet (7.62 m) of lateral travel for employees.    On or about May 5, 2020, at least one employees working at the base of a trench that was more than 4 feet in depth and was not provided a safe means of egress.
