29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:    On or about May 28, 2020, the employer failed to ensure employees worked with their feet on the platform of a UpRight SL30SL scissor lift. Employees utilized the guardrails as a ladder and climbed out of the scissor lift onto the roof to work.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  On or about May 28, 2020, the employer failed to ensure employees performing siding work approximately 12 feet off the ground were protected by fall protective devices/systems.
