29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c).  a) Hwy 280, Phenix City; On or about May 2, 2020 the employer exposed employees to cave-in hazards in that the employees were required to work in a trench measuring five feet eight inches deep, nine feet wide and eleven feet in length was dug in class B soil.  The employer failed to provide an adequate protection systems to protect from cave-ins and the excavation was sloped at steeper than one and one half horizontal to one vertical.
