Groh's Home Improvements, LLC, on or about June 3, 2020 at the address of 609 N. Clinton St. Defiance, OH: The employer did not have a plan or program to identify job site hazards which assists employees in maintaining a safe working environment. Such programs would support the employer and employees in identifying and avoiding potential hazards such as preventing falls from heights on the worksite.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:    a)   Groh's Home Improvements, LLC, on or about June 3, 2020 at the address of 609 N. Clinton St. Defiance, OH: The employer did not ensure employees had been trained in the appropriate use of fall protection such as, but not limited to, the use of personal fall arrest systems or alternative measures while performing various roofing operations, including but not limited to applying shingles. Employees were unable to identify fall hazards on the job site, exposing them to fall hazards of approximately 20ft.

Clinton St. Defiance, OH: the employer did not ensure employees were protected from falls while performing roofing activities for a residential type structure.  Employees were exposed to up to 20 ft. falls to the ground below.
