29 CFR 1910.178(l)(6): The employer did not certify that each operator has been trained and evaluated as required by this paragraph (l):  (a) On or about June 4, 2020, and at times prior, the employer did not certify that the fork lift operator had been trained.  Certification must include the name of the operator, the date of training, the date of the evaluation, and the identity of the person(s) performing the training or evaluation, and the certification shall be updated at least once every three years.  Abatement Note:  Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").

29 CFR 1910.1200(f)(6)(ii): Except as provided in 29 CFR 1910.1200(f)(7) and 29 CFR 1910.1200(f)(8), the employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the product identifier and words, pictures, symbols, or combination thereof, which provide at least general information regarding the hazards of the chemicals and which, in conjunction with the other information immediately available to employees under the hazard communication program, would provide employees with the specific information regarding the physical and health hazards of the hazardous chemical.  a() On or about June 4, 2020, and at times prior, employees working in the service department were exposed to hazardous chemicals in that numerous plastic containers labeled as diesel exhaust fluid actually contained windshield washer fluid, a can of brake cleaner was not labeled and another plastic container labelled as cleaner was actually coolant.    (b) On or about June 4 2020, and at times prior, an employee in the detail department was exposed to hazardous chemicals in that a spray bottle labeled as glass cleaner was actually Interior Cleaner and an additional container with the same Interior Cleaner was not labeled.   Abatement Note:  Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").
