29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards.  On or about June 4, 2020, at the previously addressed site, employees exposed to falls while engaged in construction work and installing siding on a residential building were not provided training to recognize the hazards of falls while working from a scaffold.  Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1926.1053(b)(7): Ladder(s) that were unsecured or not provided with slip-resistant feet were used on slippery surfaces.  On or about June 4, 2020, at the previously addressed site, employees installing siding on a residential building and engaged in other construction activities used an unsecured extension ladder equipped with rubber feet which was placed on a concrete surface with a piece of 2x4 wood placed under one foot of the ladder.  Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).
