29 CFR 1926.760(a)(1): Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems:  a)  At the construction site, employees standing atop the metal deck within a yellow-flagged area, 24-feet above the ground, were not protected by any type of a fall protection system.  b)  At the construction site, on or about May 29, 2020, employees atop the metal deck within a red-flagged area, 24-feet above the ground, were not protected by any type of a fall protection system.

29 CFR 1926.760(d)(2): Fall arrest system components used in fall restraint systems did not conform to the criteria in 29 CFR 1926.502:   a)  On or about May 29, 2020, at the construction site, employees performing steel erection activities were exposed to fall hazards of approximately 24-feet and greater when using 50-foot long and 65-foot long retractable lifelines that would not prevent an employee from free falling more than 6-feet or 1.8 meters.
