29 CFR 1910.134(f)(2): Employee(s) using tight-fitting facepiece respirators were not fit tested prior to initial use of the respirator:  a) Long Island Jewish-Forest Hills Hospital located at 102-01 66th Road, Forest Hills, NY 11375:   On or about May 13, 2020, and continuing until July 30, 2020, the employer did not provide a fit test to an employee required to wear a respirator.  The employer required employees to wear N95 filtering facepiece respirators to protect against SARS-CoV-2 while providing care to suspected and confirmed COVID-19 patients and cleaning COVID-19 patient rooms and hospital operating suites.  The employer failed to ensure a fit test was performed prior to initial use when the employee was required to wear a different respirator.
