29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a) Top of the roof, Front Fascia: On or about June 15, 2020, an employee was exposed to fall hazards of nine (9) feet to the ground below while performing roofing activities, in that fall protection was not provided.

29 CFR 1926.502(d)(17): The attachment point of the body belt was not located in the center of the wearer's back. The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head.  a) Top of the roof, Front Fascia: On or about June 15, 2020, an employee was exposed to back injuries while performing roofing activities, in that the employee attached a life line to carabiners located in the front webbing of the body harness located at his waist.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  (a) a) Top of the roof, Front Fascia: On or about June 15, 2020, employees were exposed to back injuries and fall hazards while performing roofing activities, in that the employees were not provided proper fall protection training.
