At the job site, employees were not provided eye protection when operating a masonry saw cutting bricks and were exposed to eye injuries from flying objects.

29 CFR 1910.134(a)(2):  A respirator was not provided by the employer to each employee when such equipment was necessary to protect the health of the employee: (Construction Reference 1926.103)    At the jobsite, employees who were cutting masonry stone with a Dewalt Masonry cutting saw were not protected with personal protective equipment and exposed to respiratory hazards from the dust, silica dust and particles.

29 CFR 1926.405(g)(2)(iv): Strain relief. Flexible cords shall be connected to devices and fittings so that strain relief is provided which will prevent pull from being directly transmitted to joints or terminal screws.        At the jobsite, employees used a yellow extension cord supplying power to a Dewalt circular saw where the strain/tension pulled the outer protective sheathing away, exposing employees to electrical shock and fire hazards.

At the construction site, the employer did not ensure that employees using a masonry saw to cut bricks and stones were protected from being exposed to silica with the use of engineering controls, work practices and respiratory protection.
