At the jobsite, employees were not wearing hardhats while working below other employees that were installing framing material to a residential house, exposing employees to struck-by hazards from falling objects.

At the jobsite, employees operating DeWalt nail guns while performing framing activities were not wearing eye protection, exposing employees to the hazard of being struck by flying objects.

At the jobsite, employees engaged in roofing activities at a height of over 6 feet were not protected by a fall arrest system. Employees were exposed to an unprotected fall of over 8 feet to the ground below.

The employer did not provide a training program with hazard recognition and avoidance training where part of the job is to be exposed to fall hazards at a heights over 8 feet to the ground below.
