29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a- Building Exterior (Entrance): An employee using a portable ladder to access the roof was exposed to fall hazards of 12ft as the ladder side rails did not extend 3ft above the upper landing surface. 29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:   a- Building Exterior (Entrance & Roof): An employee engaged in roofing activities was exposed to fall hazards of approximately 12 & 20 ft  as a means of fall protection had not been utilized. Condition noted or about 06/16/2020.     Tonny Construction LLC was previously cited for repeat violations of this Occupational Safety and Health standard or its equivalent standard, 29 CFR  501 (b)(13) , which was contained in OSHA Inspection Number  877483  and 1085685.  In regards to Inspection Number 877483, Citation #01, Item # 004, was issued on 03/20/2013 to a workplace located at Walter Ave, Hasbrouck Heights NJ 07604. This citation became a final order on 12/17/2013.  In regards to Inspection Number 1085685, Citation # 02, Item # 01, was issued on  01/07/2016 to a work place located at  230 Boulevard, Hasbrouck Heights NJ 07604. This citation became a final order on 11/07/2016.
