29 CFR 1910.178(m)(5)(iii): When the operator of an industrial truck is dismounted and within 25 ft. of the truck still in his view, the load engaging means shall be fully lowered, controls neutralized, and the brakes set to prevent movement.   a.) On or about 06/18/2020, at the boat repair shop's yard, the forklift operator who was within 25 ft of the truck, did not ensure that the load was lowered, the powered industrial truck's controls were neutralized and the brakes set to prevent movement, exposing employees to struck-by and fall hazards.

Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, you must report the in-patient hospitalization, amputation, or loss of an eye to OSHA.  a.) At the job site, on 6/11/2020  the employer called OSHA to report an in-patient hospitalization which occurred on 6/09/2020.
