29 CFR 1910.22(d)(2): The employer did not ensure hazardous conditions on walking-working surfaces were corrected or repaired before an employee used the walking-working surface again.  When the correction or repair cannot be made immediately, the hazard was not guarded to prevent employees from using the walking-working surface until the hazard is corrected or repaired:  (a) North side of building - On or about June 18, 2020 the employer did not ensure that the steel cover plates for drain pipes were properly secured exposing employees to trip hazards.

29 CFR 1910.303(h)(3): Energized parts of electric equipment rated over 600 volts, nominal, were exposed and the minimum clear work space was less than 3 feet wide (measured parallel to the equipment):  (a) Pool Room - On or about June 18, 2020 the employer did not ensure the 5 live electrical panels had a 3 foot clearance in front of them, exposing employees to fire hazards.
