29 CFR 1926.95(a): Personal protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, was used as was necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation, or physical contact. (a) 2391 Sissonville Drive Charleston, WV 25312: On June 24, 2020 and times prior, employees were using Versigard G200SA Yellow Substrate Adhesive without the use of gloves, protective clothing, eye protection and face protection as is required by the manufacturer.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system: (a) 2391 Sissonville Drive Charleston, WV 25312: On June 24 and times prior, employees were installing rubber on a flat roof that measured approximately 15 feet edge to ground, without the use of a fall protection system.
