COMPANY-WIDE:  On or about June 29, 2020, the employer had not developed and implemented an exposure control plan designed to eliminate or minimize blood-borne pathogen exposure of certified nursing assistants and patient care associates who care for clients in their homes.

29 CFR 1904.4(a): The employer did not record each work-related fatality, injury, or illness case that resulted in the general recording criteria on the OSHA Form 300 or equivalent.    Abatement Certification and Documentation is required  COMPANY-WIDE:  On or about 29 June 2020, the employer did not develop at least three years of injury and illness logs as required by 29 CFR 1904.4(a)(1) through 1904.4(a)(3).
