29 CFR 1910.157(g)(2): The educational program to familiarize employees with the general principles of fire extinguisher use and the hazards involved with incipient stage fire fighting was not provided to all employees upon initial employment, and at least annually thereafter:  A.)  Hy-Tech Machine, Inc. dba Power Transmission Group, 260 Meter Street, Punxsutawney, PA  15767, on or about June 24, 2020 and times prior thereto:  Employee were provided with, and permitted to use, portable multi-purpose fire extinguishers.  Employees have not had annual fire extinguisher refresher training within the last year.  ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below).  This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.

29 CFR 1910.157(c)(1): Portable fire extinguishers were not mounted, located and identified so that they were readily accessible without subjecting the employees to injuries:  A.)  Hy-Tech Machine, Inc. dba Power Transmission Group, 260 Meter Street, Punxsutawney, PA  15767, on or about June 24, 2020 and times prior thereto:  A Badger multi-purpose dry chemical fire extinguisher (M/N:  10M-1,  S/N:  WS-14289) was setting on the floor at the base of a column, and was not mounted to the column.  B.)  Hy-Tech Machine, Inc. dba Power Transmission Group, 260 Meter Street, Punxsutawney, PA  15767, on or about June 24, 2020 and times prior thereto:  A Badger multi-purpose dry chemical fire extinguisher (M/N:  10M-1,  S/N:  WS-14875) was setting on the floor at the base of a column, and was not mounted to the column.  ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below).  This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.

Hy-Tech Machine, Inc. dba Power Transmission Group, 260 Meter Street, Punxsutawney, PA  15767, on or about June 24, 2020 and times prior thereto:  Columns throughout the production area were marked with fire extinguisher signage, and were not equipped portable fire extinguishers. Hy-Tech Machine, Inc. dba Power Transmission Group, 260 Meter Street, Punxsutawney, PA  15767, on or about June 24, 2020 and times prior thereto:  Employees use personal protective equipment including, but not limited to, safety eyewear.  The company has not developed a personal protective equipment hazard assessment for the work operations and associated hazards at this location.  ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below).
